Are you tired of filling out a new Permission Slip for every campout when your information does not change?  To help make it easier for you, we are providing a blank copy of the Permission Slip in Microsoft Word Format.  Please save CLICK HERE and SAVE to your computer, please do not try to OPEN directly.  This will allow you to enter in all the static information so you will only need to update the campout description and location in the future.

NOTE:  Any changes made to the Troop 2806 permission slip will automatically be in place even if it is not shown on the version of the permission slip you turn in.  It is your responsibility to make sure you have the current version of the permission slip at all times.  The troop will not be held responsible for missing information on the permission slip.

REMINDER:   Electronics are NOT allowed on Troop 806 campouts unless otherwise stated.  Please leave all electronic devices at home.  The will be confiscated!

MEDICATION:    If any scout needs to take medication of any kind during the campout, the medication must be hand-delivered by the parent to the “Medicine Man” before leaving WUMC.  All medication must be in zip lock bag, clearly labeled with administering instructions—NO EXCEPTIONS!!   UP-TO-DATE HIPPA Form is REQUIRED.

PERMISSION SLIPS AND FEES:  Are due at the BEGINNING of the troop meeting. Scouts, not parents, should have these items ready to turn in as soon as they arrive for the troop meeting. The money for the purchase of patrol food is redistributed to the patrols before the end of the meeting. Late permission slips and fees create accounting and registration difficulties for the troop scribes.

Monthly campouts are not an expenses that can be deducted from your scout account unless otherwise stated in advance.
